Perham Health offers financial assistance to patients who are unable to pay for emergency or medically necessary care.
Availability of Financial Assistance
You can ask for help with your bill at any time during your visit or billing process. You may be able to receive financial assistance if you do not have insurance, are underinsured, or if it would be a financial hardship to pay the out-of-pocket expenses for services received at Perham Health.
Financial assistance is determined by a sliding scale of total household income based on the Federal Poverty Level (FPL) found at: https://aspe.hhs.gov/poverty-guidelines. If you and/or the responsible party’s income is at or below 200% of the FPL, you will have no financial responsibility for the care received at Perham Health. If you fall between 200% and 400%, you may be eligible for discounted rates for the care given.
How to apply
To apply for Perham Health financial assistance you can go online at perhamhealth.org and search financial assistance. You can also receive an application in person at any Perham Health facility, or by calling Patient Account Services at (218) 347-4500.
You are responsible for providing information timely about your health benefits, income and other paperwork required.
Perham Health will uphold the confidentiality and dignity of each patient, and any information submitted for consideration of financial assistance will be treated as protected health information under the Health Insurance Portability and Accountability Act (HIPAA)
No more than Amount Generally Billed (AGB)
No person eligible for financial assistance under the financial assistance plan will be charged more for emergency or medically necessary care than the amount generally billed to patients who have insurance coverage for such care.

Perham Health provides interpreters upon request of the patient or patient’s companion to accommodate either language or disability needs.
