Perham Living has been actively involved in transforming its traditional nursing facility into a true home for residents since 2001. Using the Household Model as a guide, six households of 16 residents each with all services decentralized to the household, have been developed.
Our employees have experienced first hand the transition and understand the stresses and challenges involved in changing the long-term care culture.
The lessons that staff have learned can be helpful to all facilities. We understand that every facility is unique and may require different approaches to meet needs. Our award-winning leadership team has presented at numerous national and international conferences, including the Pioneer Network, Institute for Family-Centered Care, and Aging Services of Minnesota. We are committed to helping others find “their own way HOME,” convinced that changing the culture of long-term care is possible in any setting.
Experienced staff are available to work with facilities to develop a tailored approach to their specific needs as they relate to culture change.
Education sessions specific to your needs, can be arranged. Areas of expertise include, but are not limited to the following:

- Household Model of care
- Cross-training for staff
- Organization redesign
- Self-directed work teams
- Communication skills and conflict management
- Defining household roles
A two-day experience provides an opportunity to observe direct care staff as they work in the household.
Tours of households are two hours long and include a description of the model of care and process used by Perham Living to achieve culture change.
