The health of a community depends on many different factors; environment, education and jobs, access to and quality of healthcare, and individual behaviors. Everyone has a stake in community health. Within that framework of understanding, the purpose of the Perham Community Health Needs Assessment provides a comprehensive view of the community’s health.

The information gathered in this report illustrates what we know about physical, social, and behavioral factors for health in the community; along with health outcomes related to those factors.
