Health and wellness is never far from the minds of most. As we grow older, decisions about our abilities and those of our loved ones can be hard to make. In November, Perham Living celebrates Home Care Month as a wonderful means to providing dignity and respect to all in need of a little extra help.

What does home mean to you?
For Perham Living Home Care staff, we hear that home is where you live, where families have grown, pets play, reminders of the past reside, a safe haven, and comfort zone. Most importantly, a place where you can just be yourself. Along with this mindset, framing your health care needs to the home you live in greatly impacts your overall wellness and ability to remain at home.
Because of this, we understand why Home Care is growing in demand and widely embraced by patients. The benefits of receiving care at home result in a longer, happy life. Knowing this, the visiting nurses, therapists, and home health aides, provide emotional support through any stage the patient and caregiver are going through. Also, the social interactions by these professionals fill the void of isolation that can occur from a prolonged recovery or homebound condition.
With these visits, staff may provide services similar in skill to those of an acute care setting or transitional care unit. Additionally, complete home safety evaluations and recommendations are given. Along with encouraging individuals to maintain independence and regain fundamental living habits round off our visits. This guidance helps to prevent emergency room visits and hospitalizations. In turn, the cost of home care is much less than that of acute or long term care in a health care facility.

Individualized care
The individual’s professional health care services are driven by the needs of the patient and directed under the provider’s built plan of care. While a few health care services can be indefinite, such as chronic care management to prevent long term or assisted living placement, other services can be added as needed. Additionally, lab draws, specialized therapies, blood pressure checks, medication management, rehabilitation, neuro/stroke care, and wound care are a handful of the services available through Perham Living’s Home Care. However, these services are not limited to the senior population. For example, newborns on up are able to receive home health care visits for services recommended by their provider.
A nurse, therapist and/or home health aide may stop by at the beginning of your care. They give education, specialized care and treatments, and direction for improving health to eliminate re-admittance to the hospital or emergency room. Thus, our goals of the patient being at home during recovery or for maintaining independence are echoed by the support our staff brings to the patient and their caregiver. Most importantly, the passion for people and witnessing improvement in each patient, brings joy to the staff at Perham Living Home Care.
