World’s Best Hospitals 2025 – Newsweek & America’s Best

We are thrilled to be named among the World’s Best Hospitals for 2025 by Newsweek, in collaboration with America’s Best. These organizations have identified the top 412 general medicine hospitals in the United States—both large and small—based on a combination of patient experience scores, reported quality measures, and patient safety data.

Healthgrades Outstanding Patient Experience Award

We are also proud recipients of the Healthgrades Outstanding Patient Experience Award, which honors hospitals that go above and beyond to deliver exceptional experiences for their patients. This award is based on HCAHPS survey data from the Centers for Medicare and Medicaid Services (CMS) and recognizes organizations that listen to patients, respond to their needs, and create a culture of trust and respect.

Minnesota Department of Health Zero Deficiency Rating – Perham Living

Perhaps one of the most extraordinary achievements came this month when Perham Living received a Zero Deficiency Rating during a recent comprehensive survey by the Minnesota Department of Health. For three days, multiple surveyors reviewed every aspect of our operations—and found no deficiencies.

To put this in perspective, skilled nursing facilities are the most heavily regulated industry in the United States, even surpassing nuclear power. Achieving a zero-deficiency outcome is extremely rare and speaks to the exceptional quality, attention to detail, and culture of excellence at Perham Living.

“A zero-deficiency survey is virtually unheard of in our industry,” said Hofius. “It’s a powerful statement about the care, professionalism, and commitment of our team at Perham Living. I couldn’t be more proud of what they’ve accomplished.”
