MyChart – Quick. Convenient. Secure.

MyChart allows our patients to schedule appointments, check lab results, send messages to their providers, renew prescriptions, and much more, easily from a computer or smartphone. More than 60% of Perham Health patients use MyChart for their healthcare needs. If you are not in that percentage of users, we want to show you all that could be at your fingertips. MyChart improves the patient experience and provides convenient access to your healthcare wherever you are.

You can sign up for MyChart by visiting www.perhamhealth.org and selecting MyChart in the blue navigation bar at the top of your screen. Next, click the ‘Request Access for Yourself’ button followed by ‘Activate Your Account’.

Communicate with your care team

When you have a medical question, sending your healthcare provider a message through MyChart is one of the most convenient ways to communicate. It doesn’t matter what time it is — or if you’re in your office or the grocery store parking lot — you can still reach out with a MyChart message to start a conversation with your provider. You’ll see a list of your providers who use messaging. Click on the provider you want to reach, write your message, and hit send.

Receive test and lab results

No more waiting for a phone call or letter – view your results as soon as they are available. If you have questions about a result, you can message your provider right from here. You can also view test results from the past.

Schedule appointments

Whether you prefer an in-person visit or a virtual appointment, you can view open appointment slots based on the type of visit you need and the provider you need to see and then directly schedule an appointment.

Request prescription refills

MyChart makes managing your medications easier. A list of all prescribed medications is viewable as part of your medical record. Send a message to your clinic team if you have a prescribed medication you would like renewed.

Access your family’s records

With family access, you can view your loved one’s online medical record, schedule and manage appointments, renew prescriptions, and communicate with their care team.
